PHYSICIAN’S CERTIFICATION
FOR QUALIFICATION TO USE A CROSSBOW
(Oklahoma Statute 29, 5-209)
The Director of the Department of Wildlife Conservation may permit the hunting or taking of wildlife by use of a crossbow by persons, otherwise qualified, who have a permanent disability to the extent that they cannot physically use a conventional longbow, as certified by a physician licensed to practice medicine in this state or in any state which borders this state. A person who qualifies to use a crossbow shall have in their possession while in the field, written evidence of such certification.
Any crossbow user must still adhere to the Wildlife Department rules and regulations during the archery deer season.
To use this form as your written evidence in the field, you must have a licensed physician sign and complete the form certifying that you meet the above requirements to use a crossbow.
