The Great Plains Trail of Oklahoma Site Nomination Form

If you would like nominate a site to be added to the Great Plains Trail of Oklahoma, please download the word document and fill it out. You will need to include information about the site in order to evaluate the site against the criteria. Mail the completed form to the address below. Your application will be  reviewed by the state steering committee. You will then be notified about your site status.
